The Respite process is currently as follows:

 

When you (foster parent(s) need respite, we request at least 1 week advance notice to your case manager.

The case manager in turn will notify their supervisor, who will identify the most available and appropriate options.  (Factors that could influence respite is depending on the type of issues your current placement has. i.e. if placed child has sex offender issues it would be too risky to placed with younger children.)

Martha (receptionist) is given the list of parents identified by the supervisor to contact to request their services for respite. 

Upon respite parent acceptance, you as the primary parent are notified and given contact information to arrange for drop off.

 The voucher needs to be completed and given to the respite parent at drop off.  To save time for subsequent respites, information that may be consistent for longer period of time can be pre-populated duplicates copies can be made to only fill in updated information.  If you have a computer download this form and save it on your hard drive with information that will remain constant for longer periods of time.

 On the form itself it ask for the child’s name, date of birth (DOB), Primary Parents (You) address and phone number.  (pre-populate and save or make duplicate copies).

 Next it ask for the Respite Parent.  Date & Time respite will Start and End.

 Next you see Planned Respite, Administrative Respite, and Other reimbursement.

Planned respite is your earned respite days per every 30 days of service.

To use your Planned Respite just check the box or space before Planned Respite.

Next is Administrative Respite, which is used to transfer funds from you as the Primary Parent to the Respite Parent to be paid at the beginning of each month with your reimbursement. 

To use Administrative Respite to pay out of your reimbursement check box or space before Administrative Respite.

Last is Other reimbursement for respite arrangements made between foster parents outside of the normal $50 per day compensation.  Some parents may need respite for the day only.  Arrangements could be made for a lower compensation that is agreed upon by both primary and respite parent, i.e. If respite parent agrees to provide respite for a child for only $15 per night.  You would indicate that on the line below.  These funds will also be transferred at reimbursement.

To use Other reimbursement check box or space before Other reimbursement.

And write out arrangements made below.

Following it list Case Manager and cell phone, and Legal Custodian fill this information out also for the respite parent will know who to contact if there is an emergency or custodian is attempting to contact child, the respite parent has a name to verify. (pre-populate and save or make duplicate copies).

Numbers  1 – 6 is information on the child for the respite parent to reference so they will know how to properly supervise child during respite.  For most children this information may not change much.  (pre-populate and save or make duplicate copies).

Number 7 may change from time to time so I recommend this be update each time respite is requested to make sure right dosages are given.

Once this is completed keep a copy for yourself and give a copy to the respite parent with your signature and date on the form. 

Respite parents are to send this form in upon completion of Respite signed.  The case manager will sign to complete the process and send to Loadstone for payment.
